- You must submit your Claim Form online no later than Tuesday December 8, 2026. or mail your completed paper Claim Form so that it is postmarked no later than Tuesday December 8, 2026.
- You must complete and mail your written Request for Exclusion so that it is postmarked no later than Monday November 9, 2026.
- You must file your objection(s) and/or notice of intent to appear at the Final Approval Hearing so that it is postmarked or submitted no later than Monday November 9, 2026.
- The only way to receive Benefits from this Settlement is to submit a valid and timely Claim Form December 8, 2026 EXCLUDE YOURSELF FROM THE SETTLEMENT If you exclude yourself (or “opt out”), you will not be bound by the terms of the Settlement and you keep the right to sue the Defendants about the claims resolved by this Settlement.
- To exclude yourself from the Settlement, you must submit a written request for exclusion that includes the following information: Your full name, address, and phone number(s); An unequivocal statement of your intention to exclude yourself from the Settlement, “I/We request to opt out of the Settlement in McGrath v. Suffolk County, et al., Index No. 608155/2016. (Deadline: November 9, 2026)
