You'll need

  1. The only way to receive benefits or payments from this Settlement is by submitting a valid and timely Claim Form. (Deadline: September 30, 2026)
  2. You must have spent the time on tasks related to the Data Security Incident.
  3. If you claim for either Ordinary Documented Out-of-Pocket Expenses and/or Extraordinary Documented Out-of-Pocket Expenses, you will need to submit proof, like receipts, to show how much you spent or lost.
  4. You can also provide notes or papers you made yourself to explain or support other proof, but those notes or papers alone are not enough to make a valid claim. (Deadline: September 30, 2026)
  5. Your proof or notes should show that your expenses were because of the Data Security Incident.
  6. To opt out of the Settlement, submit a written Request for Exclusion to the Settlement Administrator postmarked by September 16, 2026.
  7. To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by September 16, 2026.
