- If you are a Class Member and wish to receive Settlement Benefits then you must submit a claim form by September 28, 2026.
- The only way to receive benefits or payments from this Settlement is by submitting a valid and timely Claim Form. (Deadline: September 28, 2026)
- To opt out of the Settlement, submit a written Request for Exclusion to the Settlement Administrator postmarked by August 28, 2026.
- To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by August 28, 2026.
