  3. You can also provide notes or papers you made yourself to explain or support other proof, but those notes or papers alone are not enough to make a valid claim. (Deadline: October 22, 2026)
  4. Your proof or notes should show that your expenses were because of the Data Incident.
  5. To opt out of the Settlement, submit a written Request for Exclusion to the Settlement Administrator postmarked by October 7, 2026.
  6. To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by October 7, 2026.
