2. The only way to receive benefits or payments from this Settlement is by submitting a valid and timely Claim Form. (Deadline: August 24, 2026)
3. To opt out of the Settlement, submit a written Request for Exclusion to the Settlement Administrator postmarked by August 24, 2026.
4. To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by August 24, 2026.
