You'll need

  1. Submitting a valid Claim Form is the only way you can receive Credit Monitoring, Expense Reimbursement, or an Alternative Cash Payment. (Deadline: July 20, 2026)
  2. If you prefer, you can download the Claim Form here and mail it to the Settlement Administrator. (Deadline: July 20, 2026)
  3. To exclude yourself from the Settlement, you must send a letter by mail stating that you want to be excluded from the Settlement in Amy Walker v. New River Electrical Corp., Case No. CV25-126091. (Deadline: June 19, 2026)
  4. Settlement Class Members with expense reimbursement claims must submit documentation supporting their claims.
  5. Settlement Class Members must submit a Valid Claim Form, but no documentation is required to make a claim. (Deadline: July 20, 2026)
  6. To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by June 19, 2026.
