You'll need

  1. Settlement Class Members with Documented Out-of-Pocket Losses must submit plausible documentation supporting their claims.
  2. Settlement Class Members must submit a valid and timely claim, including necessary supporting documentation, to the Claims Administrator.
  3. Settlement Class Members must also have made reasonable efforts to avoid, or seek reimbursement for, the loss, including but not limited to exhaustion of all available credit monitoring insurance and identity theft insurance.
  4. Settlement Class Members who wish to claim credit monitoring must do so by the Claims Deadline on September 28, 2026, by selecting the credit monitoring option on the Claim Form.
  5. To opt out of the Settlement, submit a written Request for Exclusion to the Settlement Administrator postmarked by August 28, 2026.
  6. To object to the Settlement, submit a written objection to the Court (with copies to Class Counsel and Defendants' Counsel) postmarked by August 28, 2026.
